چکیده

A 54-year-old diabetic, hypertensive smoker (60 pack years) with chronic kidney disease Stage V and on dialysis, presented with hoarseness and weight loss. His hemoglobin was 9 g/dL, erythrocyte sedimentation rate (ESR) 60 mm at the first hour, blood urea 7 mmol/L and serum creatinine 70 μmol/L. Computed tomography (CT) of the chest showed a mass in the hilum of the left lung and enlarged mediastinal and cervical lymph nodes. Meanwhile, he developed a high-grade fever, low back pain and painful fixed flexion of both hip joints. The Xrays of both hip joints and the lumbosacral spine were normal. In clinical assessment, bronchogenic carcinoma with skeletal secondaries was suspected. The results of the 18-F Fluoro deoxy glucose (FDG) Positron Emission Tomography (PET)-CT scan showed increased tracer uptake in the mediastinal and cervical lymph nodes, left lung hilum, dorsal vertebrae, bilateral iliacus muscles, bilateral hip joints and acetabulum of the right femur (Figure 1). Fine-needle aspiration of the left iliacus muscle revealed pus with plenty of polymorphs. Tuberculosis (TB) polymerase chain reaction evaluation of the pus was positive. He was initiated on antituberculosis treatment. Mycobacterium TB (MTB) was grown after 6 weeks. The diagnosis was multifocal skeletal TB (MFSTB). MFSTB is an entity where osteoarticular lesions occur simultaneously at two or more locations [1]. MFSTB constitutes less than 5% of skeletal TB. MFSTB is more common in those who are immunocompromised, or on hemodialysis [1]. The radiological evaluation reveals multiple destructive lesions in the bone, which may be confused with secondary or primary osseous lesions. Hence, the diagnosis of MFSTB is ultimately confirmed on histopathology or on microbiological studies. In our patient, lymph nodes and multiple skeletal sites were involved. Microbiological evaluation confirmed MTB. In conclusion, in the presence of multiple skeletal lesions in patients on dialysis, MFSTB should be considered in the differential diagnosis. MFSTB is a great mimicker of malignancy and hence requires early confirmation.

Multifocal skeletal tuberculosis: A case report

Tuberculosis (TB) of the musculoskeletal system is a rare clinical condition. Multifocal bone involvement is extremely rare and difficult to recognize. Thus, due to the diverse and atypical clinical manifestations of multifocal skeletal TB, the disease is easy to misdiagnose. In the present study, a rare case of atypical disseminated multifocal skeletal TB was reported, which exhibited uncommon...

Multifocal Skeletal Tuberculosis Mimicking Langerhans Cell Histiocytosis in a Child: a Case Report With a Long-Term Follow-Up

INTRODUCTION Multifocal skeletal tuberculosis is a rare condition that may masquerade as Langerhans cell histiocytosis, especially in children. CASE PRESENTATION We report a case of multifocal osseous tuberculosis in a 5-year-old female patient admitted to our hospital with a complaint of low back pain but no history of respiratory symptoms or malaise. Radiological findings included vertebra ...

Multifocal skeletal tuberculosis involving the lumbar spine and iliac bone, mimicking a malignant bone tumour: a case report.

ABSTRACT Multifocal osteoarticular tuberculosis is uncommon and accounts for approximately10% of all cases of osteoarticular tuberculosis. Concomitant vertebral and iliac bone tuberculosis has not been reported in the literature to date. We report one such case for its unusual clinical and radiological presentation, which had presentation more similar to a malignant bone tumour than an infectio...

Multifocal skeletal tuberculosis in an immunocompetent patient: a case report

BACKGROUND The occurrence of multifocal skeletal involvement in immunocompetent patients is rare, even in countries where tuberculosis is endemic. Multifocal skeletal lesions may occur as a result of hematogenous dissemination from another primary focus such as cervical lymph nodes, lungs, tonsils or gastrointestinal tract. CASE PRESENTATION We present a 59 year-old man with a history of inte...

Diagnostic role of whole body bone scintigraphy in atypical skeletal tuberculosis resembling multiple metastases: a case report

INTRODUCTION Osseous tuberculosis can be present with unifocal or multifocal bony involvement. Although multifocal involvement of the skeletal system in areas where tuberculosis is endemic is not a rare presentation, its exact prevalence is not well known. A case of atypical skeletal tuberculosis mimicking multiple secondary metastases on radiologic and scintigraphic imaging is presented to emp...
